How to download and view Adobe Acrobat forms/files

What is the purpose

On the preceding page you'll find a list of forms. These forms are PDF (Portable Document Format) files. You may download a copy or copies of any form you wish, for your convenience. Simply print, fill out and return or fax to the address at the back of the form.

How to download the Forms

Simply click your right mouse button on a selected form. Choose the option "Save as ", "Save target as" or "Save link as" and keep note of where the form is saved for later use.

Please note that some of our forms are now available as dynamic PDF files and can be completed directly on screen.

How to View and Print these Forms

Adobe Acrobat Reader is a program which reads and prints PDF files. The forms that can be filled out on screen have a minimum requirement of Acrobat Reader Version 7.

To view or download the forms:

• Double-click on the Acrobat Reader Software.
• Click on the File menu.
• Select the Open option.
• Retrieve the downloaded forms.

To complete the forms on-screen:

• Click into the box you wish to complete.
• Type the necessary information.
• Once printed, please remember to sign the form.

To print the forms:

• Click on the File menu.
• Select the Print option.
