Notes:
- You are providing information required by the NFP Act. Note that both the NFP Act and the Privacy Act allow this information to be disclosed to the public. It will be stored in personal information bank number IC/PPU-049.
- Each year every corporation must file an annual return within the 60 days following the corporation's anniversary date. The anniversary date is the date the corporation was incorporated, amalgamated or continued under the NFP Act. It can be found on the corporation's certificate of incorporation, amalgamation or continuance or on the Corporations Canada online database.
Item 1- Corporate name
Indicate the corporate name.
Item 2 – Corporation Number
Indicate the corporation number. You will find the corporation number on your certificate of incorporation, amalgamation or continuance. You can also find it using the Corporations Canada online database.
Item 3 – Year of filing
Indicate the year for which the annual return is being filed. The information provided must be as of the anniversary date for the year for which the annual return is being filed.
Item 4 – Date of last annual meeting of members (YYYY-MM-DD)
Indicate the date of the last annual meeting of members or the date of the last written resolution in lieu of a meeting. A written resolution is a written record of decisions that is signed by all of the members who are entitled to vote.
Item 5 – Is the corporation a soliciting corporation?
A "soliciting corporation" is one that receives public donations and/or government grants in excess of $10,000 in a single financial year.
Item 6 – Declaration
This form must be signed by an individual who has relevant knowledge of the corporation and who is authorized by the directors. This includes:
- a director of the corporation
- an authorized officer of the corporation
- an authorized agent.
Please print the name and indicate the telephone number of the signatory.
