Declaration, Title and Description
A declaration that the design was not, to the proprietor's knowledge, in use by any person other than the first proprietor at the time the design was adopted by the first proprietor is required.
Enter a title identifying the finished article or set in respect of which the registration of the design is requested (For example, "lighter," "chair," "spoon," "T-shirt."). The article should be identified by its common name generally known and used by the public. See paragraph 6.4.4 of the Industrial Design Office Practices for more information.
The description must indicate what the design features are and whether the design relates to the appearance of the article in its entirety or to the appearance of a portion of the article.
Where the design relates to the appearance of a portion of the article, the description must indicate what the relevant portion is.
The description must make clear which of the visual features (of the article or relevant portion thereof) shown in the drawings comprises the design, i.e. whether the design consists of all of the visual features or only certain specific features such as the shape.
It is suggested that figure references describing the views shown in the drawings be included.

Enter the name and address of the applicant. If the applicant is an individual, the family name and the first name must be provided. If the applicant is some other legal entity, such as a corporation, the name of the legal entity must be entered. A complete mailing address is required, including the street name and number when available.
Drawings or photographs of the design are required. See paragraph 6.5 of the Industrial Design Office Practices for more information.
If the applicant has an agent, the agent's name and complete mailing address, including the street name and number when available, is required. If this section is completed, the certificate of registration and any correspondence relating to the application will be sent to the agent rather than the person named as the applicant.
Representative for Service
If the applicant's residence and place of business is not in Canada, the name and complete mailing address, including the street name and number when available, of a representative for service in Canada is required.
In order for an applicant to make a request for priority based on an earlier filed application, both the request for priority and all the priority information must be filed within a period of six months from the date of filing of the foreign application.
A priority request must include the name of the country in or for which the application was filed; the foreign application number; and the filing date of the foreign application.
Information and Additional Services
Number of additional drawings requested:
This amount is calculated automatically upon submission of the drawings/photographs. There is a charge of $10.00 per additional page of drawings/photographs over 10 pages.
Accelerated examination requested:
Upon payment of the $500.00 fee, this application will be examined on an expedited basis.
Payment of maintenance fee requested:
Upon payment of the $350.00 fee, this application will have a pre-payment of the standard maintenance fee which will extend the term of registration to 10 years from the date of registration.

Work In Progress
All partially completed applications or those that are completed but not yet filed with CIPO remain here. Any unfiled application can be edited, deleted or sent to "Cart" in order to be submitted to CIPO. Completed registration applications that are ready to be filed with CIPO can be added to the cart by selecting the "Add to Cart" button.
This contains the list of registration applications ready to submit for payment.
To proceed with payment choose the method of payment by selecting the "Pay with Credit Card" button or the "Pay with Deposit Account" button.
To proceed with payment by credit card:
-Enter the cardholder name.
-Enter the 15/16 credit card number without spaces.
-Enter the expiry date of the card in the MMYY format as shown on the card.
-When all fields are complete select "Pay with Credit Card". The "Payment Processing" page will be displayed.
-DO NOT select the "Back" button or close the browser (the transaction will be lost).
-Once the payment has been processed, the transaction receipt will be displayed.
-To print a copy of this page for your records, select the "Print the page" link at the top right of the page.
To proceed with payment by deposit account:
-Select the deposit account from the drop-down list.
-Select "Submit". The "Confirm Deposit Account Transaction" page will be displayed.
-Verify that the details are correct and select "Confirm". The "Completing Payment Transaction" page will be displayed. Do not use the browser's "Back" button.
The "Electronic Payment Confirmation" page will confirm the details of the payment and provide an official payment receipt.
